• Buddy Poppy Order Form This is the new form for 2023 which reflects the increased prices from National.
   
• Disaster Relief Donation - Use this form to make a donation to the Georgia VFW Disaster Relief fund. (PDF)
   
• VFW National Home Donation - Use this form to record your donations to the VFW National Home for Children. This is the same form that is in the Commander's Yearbook. (PDF)
   
• VMS Donation Form - Use this form to record your Donation to the Veterans and Military Support (VMS) program. (PDF).
   
• We Care Donation Form - Use this form to record your donations to the Department of Georgia's We Care Fund. Money in this fund helps veterans in the State of Georgia.
